Even a regular citizen might be asked to fulfill some great destiny. But they don't have too, I don't think its strange at all for a pharmacist to visit the dockyards looking for exotic ingredients that might come off the boats, and if by chance the captain offers to take them along to an island teaming with flora and fauna heretofore un-encountered by said pharmacist then I would think that they would jump at the the chance to explore.
You don't need to start the main quest to go to Solstheim, just ask the captain at the Windhelm docks for 500 gold.
